Reason for Survey: 

Enhancing Education Through Technology Grant (EETT) requires completion of the survey by 90% of teachers and administrators at each school site. 

The District also uses the survey to assist it in staff development planning for the next school year.  The planning incorporates, as much as possible, the needs identified in the survey.

The District's Technology Plan also uses the survey to show that the technology skills of teachers and administrators are growing from year to year.

The District will not use your individual survey for any evaluation purposes.

Survey Completion:

All surveys must have a completion date greater than 3/31 for the current school year.  Please do not start the survey before April of the current year.

All surveys must be completed and/or updated by the end of the current school year.

EdTechProfile ID and Password Problems

 

Case Sensitive Password

The password is case sensitive.  If you received your ID/Password information from the district database, I may have made an error in the case, or the case of the letters may not have been clear.  Try the following:

If the first letter of the password is lower case, try upper case.

If the first letter of the password is upper case, try lower case.

If the password is all lower case, try all upper case.

If the password is all upper case, try all lower case.

More than one ID and Password

If more than one user ID and/or Password are listed from the district ID/Password database try the following:

ID 1 with Password 2

ID 2 with Password 1

CTAP User Name

You must use the same first name and last name that you used when you created the account.  For example, if I created my account with Robert, I cannot log in with Bob.  Try variations of your first name.

Login Help

The survey does have a help feature that utilizes the Password Hint.  If you set up a Hint, you may be able to use it to get your account information. At the Login window, click Forgot your ID Number or password?

EdTechProfile Survey Instructions

All teachers must complete or update My Technology Assessment Profile:  This is the only survey that needs to be completed.

Important:  All surveys must be updated/completed by the end of the current school year.  Not completing the survey may hold up your end of year check-out.

The survey may be taken on any computer with Internet access, including your home computer.  You do not have to update or answer all the sections/categories at one time.  As you answer each section of the survey, the responses are saved as long as you click on the button Record & Go to Next Sub Category or Record & Return to Main Menu.  To ensure that all answers are saved, you must logout properly.  If you follow these steps, then you can return to the survey to complete the unanswered sections.

Once you have reached the Edtech survey site, enter your Fist Name, Last Name, ID, Password, and click Login.  If you have problems with the login, see the document EdTechProfile ID and Password Problems.

If you do not have an account, click Create New Account Now.  You will choose your own four (4) digit ID number and your own password.

Important:  Only create a new account if you have never taken this survey.  If you took it in a different district, or if you took it in college as a student, do not create a new account.  Enter your login information for your old account, then update your user profile to you current school and assignment.

Taking the Survey

All teachers must do the following:

  1. Complete or update the information in My Technology Assessment Profile.  The survey completion date must be greater than 3/31 of the current calendar year.
  2. Be sure to use the Sign Out button to leave the survey.  Clicking Sign Out will ensure that your survey entries are saved.

Status of Survey:  Even if survey status is Green, you must take the survey unless the date is greater than 3/31 of the current calendar year. Taking the survey after 3/31 means that you will only have to complete the survey once each school year, and it allows the district to meet all EETT Grant requirements.

Unless the Assessment Completion Date is greater than 3/31 of the current calendar year, you must click Retake Assessment.

You must complete or update all sections/categories of the survey.

When you complete a section, be sure to click either Record & Go to Next Sub-Category or Record & Return to Menu.

Proof of Completion:  Once you have completed/updated all parts of the survey, print a Poof of Completion.  To do this click on the survey's My Technology Assessment Profile tab, verify that the dates for each required section are greater than 3/31 of the current year, and click the browser's print button or selectFile/Print.  Save a copy of this print out for your files.
